Michael Garron Hospital
Address: 825 Coxwell Ave.

COVID-19 Assessment Centre:outpatient clinic location in D-wing, 1st floor; accessed from Mortimer Avenue. By appointment only.

Hours: 7 days/week,  12p.m. to 8 p.m.

Note: The Assessment Centre is not open for walk-in visits. Please call ahead to book an appointment: 416-469-6858. Community members with potential COVID-19 symptoms who feel they need to be assessed for COVID-19, should phone the Assessment Centre. On the call, patients will be screened for travel and contact history and symptoms, and guided to self-monitor, self-isolate at home, come to the Assessment Centre, or go to the Emergency Department. Upon arrival to the Centre, following screening, patients will be assessed by a physician and/or nurse and tested for COVID-19 if the assessment indicates that it’s required. Only patients who meet certain criteria will be tested for COVID-19. The test is similar to that of other respiratory illnesses and involves taking a sample from the back of the nasal cavity with a swab. Toronto Public Health will follow up with those who test positive for COVID-19.

North York General Hospital
Address: Hospital Emergency Department, Ambulance Bay, 4001 Leslie St.

Hours: 7 days/week; 11 a.m. to 11 p.m.

Note: Patients will be screened for travel, contact history and symptoms upon arrival to the ED and if appropriate will be directed to the Assessment Centre through a separate entrance. Only patients who meet certain criteria will be tested for COVID-19.

Scarborough Health Network – Birchmount Hospital
Address: 3030 Birchmount Rd.

Hours: 7 days/week; 9:00 a.m. to 6:00 p.m.

Note: Pre-register online if you have symptoms including fever (over 38° Celsius), and/or cough (new or worsening), shortness of breath; AND you have been exposed to COVID-19 within the last 14 days before your symptoms began, through travel outside of Canada, close interactions with someone who has COVID-19, close interactions with someone who has symptoms of COVID-19 and recently travelled outside of Canada or to an impacted area within Canada.

Women’s College Hospital

Please note: Before coming to the Assessment Centre, take the online self-assessment or call Telehealth. Children under 2 will not be admitted to the Assessment Centre. Enter through the Northeast entrance on Grosvenor St.

**Please be advised that the eligibility for testing at the assessment centres’ is often limited and appointments are often required.  It is therefore strongly recommended that you if you are considering to going, check their specific website and/or book an appointment.
